Community Transition Liaison Program

The Community Transition Liaison Program (CTLP) is designed to assist nursing home residents (age 22+) and their families overcome barriers to discharging to the community.  Our CTLP visits nursing homes in our community weekly to increase awareness and visit with current residents enrolled in our program.

The CTLP team will provide assistance and coordination with discharge planning, including connecting residents to state programs, local community supports, and will assist the resident in mitigating issues that may impact their ability to successfully transfer to the community.

The CTLP team can assist residents return to their own home, assist with housing applications or help with finding residential settings to best suit the needs of each resident.
