Since 1977 HESSCO (Health and Social Services Consortium, Inc.) has coordinated care services and provided support, information and referral for older adults, individuals living with a disability and their caregivers and families in the Massachusetts towns of Canton, Dedham, Foxborough, Medfield, Millis, Norfolk, Norwood, Plainville, Sharon, Walpole, Westwood and Wrentham. HESSCO is located conveniently located near Route 1 in Walpole with free parking. Dependent on the position requirements, many HESSCO positions are considered hybrid with a combination of in-office and remote work.

CARE MANAGER
HESSCO is recruiting to fill the role of full-time (35 hours per week) Care Manager for our Home Care Program.
The Home Care Program helps older adults, and individuals with a disability live safely and with independence while aging in place. The role of the Care Manager is to ensure that individuals receive the most appropriate long-term care services in the proper setting based upon their individual needs. The Care Manager works cooperatively, coordinating service plans, and maintaining ongoing communication with the consumer, caregivers, informal and formal support.

GERIATRIC SUPPORT SERVICE COORDINATOR (GSSC)HESSCO has a full-time position (35 hours per week) available for a Geriatric Support Service Coordinator (GSSC). Senior Care Options (SCO) is a comprehensive health plan. SCO plans offer the opportunity to receive quality health care by combining health services with social support services. The GSSC conducts in-home assessments and provide home-based services coordination provided by a SCO plan contracted by the Aging Service Access Point (ASAP) known as HESSCO.

COMMUNITY RELATIONS MANAGERHESSCO has a full-time (35 hours per week) opportunity available for an organized and dynamic individual who enjoys networking and connecting with the community. The Community Relations Manager is responsible for HESSCO’s presence in the community and represents HESSCO at events, coordinates the recruitment of volunteers, organizes and facilitates fundraising efforts and agency events. This person will be responsible for social media, website updates, and attendance at in-person and virtual networking meetings.

SHINE PROGRAM ASSISTANTHESSCO, a non-profit social services agency, seeks a SHINE Program Assistant for up to 21-hours per week, flexible days, for the Regional SHINE Program (Serving Health Insurance Needs of Everyone). This position will work in coordination with the SHINE Program Director to provide consumers with SHINE counseling and support clerical and organizational needs of the SHINE program. If the applicant is not currently certified in SHINE, this training will be provided.
